Before you launch the CVA in your congregation, we highly recommend reading the FAQs section of this site, especially FAQ #5: Best Practices.

To launch a survey for your congregation, fill out the form below.

You will be sent an email message with a link to launch your congregation's survey, along with a password you will need to enter in order to initialize your survey.

If you don't see this email, check your Spam folder.

Also, if more than one person using the same computer, you may have to clear your cache before the next person starts to take the survey.
